All students participating in Middle Grades athletic teams are required to submit a completed sports physical/medical form.
Where to go for a Sports Physical
- Student’s pediatrician can do these at their yearly visit and just ask for a sports physical form to be signed when they are there. This would either be covered under insurance if the student has it along with an annual exam as it is done along or it could be covered with a co-pay as well.
- Piedmont Urgent Care
All locations can complete a sports physical either as walk in or appt online, it is self pay $30 and they are open 7 days a week from 7am-7pm (678-685-8100) - The Little Clinic

Call for an appt, both are open 830am-730pm, it is self pay at $45
